Coordination of care through the patient’s attending physician and/or the Advocate Hospice Medical Director and the rest of the Hospice Core Team (Nurse, Social Worker, Chaplain) Skilled Nurses Medical Social Services Spiritual Counseling Hospice Aides and Homemakers Dietary Counseling Physical, Occupational and Speech Therapists Volunteer Support Symptom Management and pain control including medications for palliative care Medical equipment and supplies related to the terminal diagnosis Ongoing education and bereavement support Core Team available for support 24 hours a day, 7 days a week
